Consents

Send the consent form to the practice tablet, or to the patient’s own phone, and keep track of who has signed. Everyone on the team can send and cancel requests; pairing tablets and changing the form belong to the owner.

The Consents page

Open Consents in the sidebar, under Resources (on a phone, under More). It shows what has been sent, what has been signed and who is still missing, in three views:

Pending
Forms sent and not signed yet, oldest first, with how and when each was sent. Waiting means the patient has not opened it; Open on device means they have started it on the tablet. The number beside Pending is how many are waiting.
Recent
The last 50 signed forms, newest first, marked Signed. A note such as Declined: Voice recording shows each optional part the patient said no to.
Missing
Active patients who have never signed the form, each with its own Request signature button. Load more shows the next 50.

A patient who signed an older version of the form is not listed under Missing. Their chart points it out instead.

Sending the form to the tablet

The practice tablet, the kiosk, stays at reception and shows the form to the patient. Before you send, it must be paired, switched on and showing its Welcome screen.

  1. Start from any of three places. On the Consents page, type at least two letters of the patient’s name in Request for…, pick them and press Request signature. Under Missing, press Request signature on the patient’s row. In the patient’s chart, press the pen icon at the top right.
  2. A small menu lists the paired tablets, for example Send to Reception iPad. Press one.
  3. The request appears under Pending and the form appears on the tablet. Hand the tablet to the patient.

A tablet that has not been heard from for a few minutes is greyed out and marked offline. With no tablet paired, the menu says No iPad paired. The form appears on every paired tablet, not only on the one you pressed: the list is there so you can see which tablets are on.

Each patient can have only one open request. A request nobody signs within 30 minutes disappears by itself.

Without a tablet, press Show QR code in the same menu. The patient scans the code with their own phone and signs the same form there, within 15 minutes. Copy link copies the same link so you can send it another way. Once they have signed, the patient can download their own copy for a few minutes.

What the patient sees

While nothing is waiting, the tablet shows the practice’s logo and name, Welcome and Waiting for a form…. When a form arrives, a card reads A form is ready for with the patient’s first name and only the first letter of the surname, for example Maria P., so whoever stands at reception sees no more than that.

  1. The patient taps Start. On the Consents page the request now reads Open on device.
  2. The form opens with the patient’s full name and date of birth, in the practice’s language. English and Ελληνικά at the top switch the whole form.
  3. For each numbered part they tap I accept or I decline. Each part is marked Required or Optional, and the form cannot be signed while a required part is declined.
  4. They sign with a finger under Sign here (Clear wipes the signature to start again) and tap Sign.

Several patients can wait at the same time, each with their own card. If a form is left open and nobody touches the tablet for 10 minutes, it goes back to the Welcome screen, and the form waits there to be started again.

After the signature

The tablet thanks the patient by first name, in the language they signed in, and confirms that the form is signed and saved with the practice. A bar counts down a few seconds, then the tablet starts over by itself: back on the Welcome screen with the practice’s logo and name, still paired and ready for the next patient. Start over now skips the wait.

At the same moment, on the practice’s computers:

  • The request leaves Pending and appears under Recent, marked Signed.
  • The pen icon in the patient’s chart turns grey. Point at it to read Consent signed and the date.
  • The signed form is kept in the patient’s profile, ready to view or to save as a PDF.

In the patient’s chart

The pen icon at the top right of every chart says where the patient stands. Point at it to read its status, or click it to act:

Amber pen
Consent not signed, or Older consent version when a required part of the form has changed since the patient signed. Click it to send the form.
Amber clock
A request is open. It names the tablet and the time the form was sent, or how many minutes a QR link has left. Click it to cancel the request.
Grey pen
Consent signed and the date. Click it if you want a fresh signature anyway.

For the full picture, click the patient’s name at the top of the chart to open Patient profile, then choose the Consent tab. It shows, for each part of the form, whether it was signed, declined or withdrawn and, under Signed forms, every form the patient has signed, with the date and whether it was signed on the tablet or by link.

The eye icon (View) shows the exact text the patient signed, with their signature. The download icon (Download PDF) saves it as a PDF.

Cancelling and withdrawing

Cancel request stops a form that has not been signed yet. You find it on the request’s row under Pending, behind the amber clock in the chart, and in the Consent tab. If the patient already has the form open on the tablet, it disappears from the screen at once.

When a patient tells you they no longer agree to a part they signed, for example sharing with other doctors or voice recording, record it:

  1. Click the patient’s name at the top of the chart and open the Consent tab.
  2. Press Withdraw beside that part.
  3. Write the Reason, which is required, and press the red Withdraw.

The part then reads Withdrawn with the date and the reason. A withdrawal cannot be undone. If the patient changes their mind, ask them to sign the form again.

Pairing a tablet

Only the owner pairs tablets. An iPad, an Android tablet or a computer with a browser can all serve as the kiosk, and a practice can have more than one.

  1. On the practice computer, open Settings, then Consent. Under Devices, press Pair a device, type a Device name such as Reception iPad, and press Pair a device again.
  2. A code appears, with the address to open on the tablet. The code is six characters, capital letters and digits. It never uses I, O, 0 or 1, so nothing can be misread, and it is valid for 10 minutes.
  3. On the tablet, open that address, add it to the home screen, and from then on always open it from that icon. How to set up the device, under the code, shows how on an iPad, an Android tablet or a computer.
  4. On the tablet’s pairing screen, type the code in Pairing code with the Latin (English) keyboard and tap Pair.

The window on the computer closes by itself and the tablet appears under Devices as Online. If the tablet says the code is invalid or has expired, press Pair a device again: the new code replaces the old one.

The owner can also Rename a tablet, and Unpair one that is lost or replaced. An unpaired tablet goes back to its pairing screen.

Changing the form

The form has four parts. Medical care and Data processing are required; Sharing with other doctors and Voice recording are optional. Every practice starts with a standard text for each, in English and Greek.

The owner changes the wording in Settings, under Consent, in Consent form. Each part has a title and a text in both languages, a Required to proceed switch, and its own Publish new version button, which saves the change. Everyone else can read the form there but not change it.

Forms already signed keep the exact text the patient saw. When a required part changes, patients who signed the earlier version show Older consent version in their chart, so you can ask them to sign again at their next visit.
