A patient’s chart
Everything about one patient in one place: history and visit notes, reports, labs, medications, vaccinations and measurements. Everyone on the team can work in a chart; the few things that depend on your role are noted below.
Finding your way around
The top of the chart shows whose it is, and each part opens something:
- Click the patient’s name to open Patient profile: their details, which you change with Edit, and their consents.
- Click the round picture beside the name for Details: AMKA, phones, address, emergency contact and a Note you can type straight into. It saves when you click elsewhere.
- Next to the name are the main diagnosis and a count such as 2 conditions. Point at the count, or tap it, to see and change chronic conditions, substance use and allergies. Click the diagnosis to change it. A red chip appears when the patient has allergies.
Under the name are the tabs. The browser’s Back button steps back through the tabs you opened before it leaves the chart.
- Overview
- The medical history, the visit notes and a column with the essentials.
- Reports
- Reports the practice writes, and documents that come from elsewhere. The number is how many there are.
- Labs
- Lab results as a table or a graph. A red number means uploaded lab reports are waiting to be checked.
- Medications
- What the patient takes now, and what they took before.
- Vaccinations
- The practice’s vaccine checklist for this patient. The number is how many are recorded.
- Measurements
- Blood pressure, pulse, weight and other readings over time.
- Prescriptions
- Shows only for doctors and the owner, in Greek practices where e-prescription is switched on.
At the right end of the tabs, small chips appear when there is something to know: the patient’s note or their visit notes, money owed, and the next appointment. Point at one to see more.
Overview: history and visits
The Overview has three columns: the Timeline of visits on the left, a document in the middle, and At a glance on the right.
The middle starts on the Main history, with sections such as Presenting complaint, Past medical history and Family history. Click into a section and type. Each section saves by itself when you click away, so there is no Save button. The owner can rename, reorder or hide these sections for the whole practice in Settings, under Clinical.
- New visit starts a visit note for today and opens it.
- Click an appointment in the timeline to open its visit note. If it has none yet, healcraft starts one, so you can also prepare for a visit that is still to come.
- Upcoming appointments come first, then past visits by year. In progress marks a visit happening now, and No note a past visit nobody wrote up.
- Main history, at the top of the timeline, takes you back to the history.
A visit note saves as you type, like the history. The bar at its foot, Recorded at this visit, lists the readings and findings taken during the visit, and Add a measurement or finding adds one. The bin at the top of the note deletes it, after asking.
At a glance keeps the essentials in view. Key numbers such as blood pressure, HbA1c and weight sit at the top, and Choose key numbers picks up to six. Below them, small tabs show the profile, recent lab results, current medications, vitals, calculated values such as eGFR and BMI, and money owed. You can record vitals and add a charge right there.
Labs
Labs has three views at the top: CBC & Biochemical, Urinalysis and Cultures, each with the number of results it holds. On the right, the table and graph icons switch between a grid of results and a graph over time.
In the table, each column is one report, with its date and laboratory, and each row is a test, grouped by panel. Values outside the reference range stand out. Click a value to correct or delete it.
To type results in yourself:
- Press Add results.
- Set the Report date and, if you like, the Laboratory (optional).
- Type a test’s name, pick it, type its value and press Enter. The cursor goes back to the test box for the next one.
- Press Save. If that laboratory already has a report on that date, choose Add to it or Create separate report.
On the graph, choose which tests to plot. The patient’s medications run underneath, so you can see each result next to the treatment at the time. Click a drug there to open it in Medications.
Letting healcraft read lab reports
Instead of typing, you can upload the laboratory’s PDF, or a photo of the printout, and let healcraft read the values. Nothing reaches the chart until you have checked it and confirmed it.
- In Labs, press AI extraction.
- Drag up to 10 PDFs or images onto the box, or click it to choose them. A password-protected PDF shows a lock and a box for its password.
- Press Extract. The window closes and healcraft reads the files in the background, so you can carry on working.
- A bar under the tabs shows Reading lab reports… and then a line such as 1 ready to review. Press Review.
The review page shows the original document on one side and what healcraft read on the other. Click a value to see where it came from. On a phone, switch between Document and Results.
- Check each report’s date and laboratory. A report without a date must be given one, or it would be filed under today.
- Click a value or a unit to correct it. Add lab adds a test that was missed.
- When a row could be more than one test, pick the right one or leave the row out. The count at the top says how many are left to resolve, and Next takes you to each one.
- With several reports, the tick and the cross on each one mark it to keep or to leave out.
After you confirm, healcraft keeps the results and deletes the uploaded files. To keep the document itself on the record, upload it under Reports as well. An upload that nobody reviews is deleted, with its files, after seven days.
A PDF can be up to 14 MB and 40 pages; split a longer one. Each file counts towards the practice’s monthly allowance, and when that runs out the upload box shows the date it resets.
Reports: writing, issuing and amending
Reports holds the reports the practice writes and documents that come from elsewhere, such as a discharge letter. Timeline lists them by date, and Folders groups them, for example by hospital stay.
- Press New report.
- Pick a template from the list, or Blank report. The ones you used last come first, and you can search by name. Check the Type and Date. If the practice has more than one doctor, choose under Signed by which of them signs it. Press Create, or double-click the template.
- The report opens as a Draft. Give it a Title and write. It saves as you type.
- A template’s fields show as boxes in the text. Tab or Enter moves to the next one.
- Press Preview PDF to see the finished document. Edit takes you back to the text, and Issue issues it.
If any fields are still blank, healcraft names them first and asks whether to Issue anyway. An issued report shows Issued and the date, Print and Download give you the PDF, and the text is locked.
To correct an issued report, open More actions (the three dots) and choose Amend. A new version opens for editing, and the issued copy stays exactly as it was. Issue the new version the same way. Versions lists them all.
To keep a document from elsewhere, press Upload, add up to 10 PDFs, images or Word files, and press Add to record. They are saved at once, and healcraft reads their date, type and issuer. Summarize with AI adds a summary and the key findings whenever you want them.
Medications
Medications shows what the patient takes. The two icons at the top switch between Table, each drug against the dates it changed, and Cards, the current drugs with past ones folded underneath. A computer starts on Table and a phone on Cards, and each device remembers your last choice.
- Press Add medication and type the drug’s name. Pick it from the Greek medicines, the active substances or the practice’s own list, or add it as you typed it.
- Type the Dosage and how often, for example 1-0-1, and the start date under Since. For a drug the patient was already taking before coming to you, tick Already taking.
- Press Enter or Add. More fields… opens the full form, with the form, route, dates and indication.
Click a drug to open its details:
- Modify
- A change of dose or schedule from a given date. The history keeps both the old and the new.
- Stop
- Ends the drug on a date, with a reason if you like.
- Resume
- Restarts a stopped drug, with the same dose or a new one.
- Edit
- Corrects a mistake. It keeps no history, so use Modify for a real change.
Import… brings in an old drug record kept in a spreadsheet. healcraft reads it and lays everything out on a review page, and nothing is added until you confirm the import there.
Vaccinations and measurements
Vaccinations is a checklist of the vaccines on the practice’s list, which is kept in Settings, under Vaccines. Mark each one Done, Not done or Recheck. For a done vaccine, set the number of Doses, and open its Note to add the date and the manufacturer or lot number.
Measurements holds blood pressure, pulse, temperature, SpO₂, weight, height and other readings. Table shows the latest readings and every reading by date, and Charts shows how they change, with blood pressure taken at the office and at home averaged separately. The buttons beside them choose how far back to look.
- In Table, Record adds a set of readings. Type the blood pressure as 120/80.
- Home diary adds many home readings at once. Paste them from a spreadsheet or type them row by row, then save.
- Readings taken during a visit, including those recorded from a visit note, show here too. Open this visit takes you to that visit’s note.
Exporting the record
Export record makes a PDF of the chart for the patient or for another doctor. The file downloads to your computer; healcraft does not send it anywhere.
- Press Export record at the top of the chart.
- Under Include, everything starts ticked. Untick what you do not want, or fill in the two dates to tick only what falls between them.
- Leave Include original documents ticked to add the uploaded files and report images. The download is then a zip file holding the PDF and the documents.
- If you like, note who it is for under Shared with (optional).
- Tick The patient has consented to sharing this record. Under it you can see whether the patient has signed consent for sharing.
- Press Export.
Every export is noted in Record disclosures, in Patient profile under Consent. Export record works even while the practice’s plan is read-only, because patients have a right to a copy of their record.
